Banner Badges

Security Badges

If you are a University of Arizona Researcher and require access to a restricted area at Banner. Please, have a manager or supervisor fill out the Request form below and email it directly to BannerResearchBadgeRequest@bannerhealth.com. A separate submission should be made for each staff member requiring a new badge and/or badge changes.

  • General access Badge requests can be completed in approximately 1 week.
  • Complex access Badge requests can take longer depending on the area and the number of areas being requested. 

Once your request is submitted your CRPD will notify you that Banner security has the request and will provide you with the ticket number.  You can pick up your badge at BUMCT Security Office Tower 4 Second Floor.

Badges expire 1 year after they are issued. An expired badge can be reactivated following the same procedure. 

As a UA employee, your UA CAT Card ID needs to be worn front facing and on top of your Banner access badge. The Banner badge is for access to Banner spaces only, it is not an ID. The UA CAT Card is your employee ID.
